Effective May 1, 2026, DPMA will require a copy of a voided check, deposit slip, bank verification letter or direct deposit form for direct deposit of DPMA benefits. This is for all initial claims.

DPMA dues are collected after taxes and DPMA provides a tax-free benefit. Therefore, DPMA does not issue an IRS W-2 or 1099 for disability payments.
